- Valid passport
- One application form & appointment letter
- One photograph (50mm x 50mm)
- Letters from employer stating dates of leave, how long you are employed, designation, monthly salary
- If going on holiday - letters of invitation from usa / Hotel vouchers
- If going on business - letters from companies in south africa and in the usa
- Itinerary
- Three month bank statement
- If students / children are travelling / letters from their schools / universities will be required
- Payment must be deposited into embassy bank account
